Field Trip Permission Slip

I, the undersigned parent or legal guardian, give permission for my child to participate in all Sky's Summer Camp field trips during the Summer 2026 season, including but not limited to:

  • Maryland Zoo in Baltimore
  • National Aquarium
  • Inner Harbor and other Baltimore adventures
  • Local pools and splash parks

I understand that my child will travel by licensed coach bus with at least two counselors per group, and that all activities are appropriately supervised. I authorize Sky's Summer Camp staff to seek emergency medical care for my child if needed, and I agree to be financially responsible for any such treatment.

I release Sky's Summer Camp and its staff from liability for any incident not resulting from gross negligence, and I confirm that my child is in good health and able to participate in age-appropriate physical activity.
